Off the top of my head, the only band I'm certain I've seen more than once is the Eagles...

...on second thought, have seen Chevelle twice.

Metallica x2
Slayer x3
Anthrax x2
Motley Crue x3 (if you count seeing Vince Neil solo at some bar on Joe Orr road back in the late 90's that I think became a country bar)

Foo Fighters x2
Poison x2    (once just out of high school, the other with Crue. they are pretty entertaining I'll have to admit)
Ozzy x2
RJ Dio (once solo, once Black Sabbath)
Cheap Trick x2
Soundgarden x2 (first time was when they opened for Skid Row at Aragon Brawlroom, second at Lollapalooza '96)
Green Day x2 (one of them was that MTV produced thing filmed at Aragon in '94....still have that on VHS somewhere)
Paul McCartney once, but it was probably the best show I've ever been to.
Rolling Stones once, at Alpine Valley '89
Guns 'N Roses x2
Alice In Chains when they opened for "Clash of the Titans" at World Music Theatre (then) around '91 or so. Not many people liked them and the bass player flipped some people off.

Sorry, this is digressing into telling stories about some of the shows...

KISS with Anthrax opening back in '88 at Holiday Star Plaza (now just Star Plaza in Merrillville). The best part of the show was some dude throwing a bottle at Gene Simmons and Gene stopping mid-song challenging the dude to come up on stage and show how much of a man he is.

The 3 times I saw them were probably in a span of 6 months. They always seemed to show up to open for other bands I was seeing. What ever happened to them? They were definitely an Incubus ripoff, but they were pretty good.

more like early System of a Down...

Singer left the band and the rest of the guys released an album under the name Mt. Helium. The album is called Faces and you should be able to find it online if you look hard...

I wonder if Mac can claim to have seen RUSH or AC/DC in five different decades.

Yes. Old 97s for sure.

There are a bunch of bands still going that I could/may see in the future that could be added.
